Pharmacy Benefit Managers:
Specifies contract terms that must be included in contract between pharmacy benefit manager & pharmacy; provides restrictions on inclusion of prescriptions drugs on list that specifies maximum allowable cost for such drugs; requires pharmacy benefit manager to disclose certain information to plan sponsor; requires contract between pharmacy benefit manager & pharmacy to include appeal process; requires pharmacy benefit manager to contractually commit to providing certain reimbursement rate for generic drugs.
