Patient Records: Requires disclosure by a licensed facility concerning individually identifiable health information transmitted to a site outside the United States; requires notice to and consent of the patient; provides for renewal and revocation of consent; provides for a consent form and contents thereof; prohibits discrimination based on refusal to grant consent.
Effective Date: July 1, 2006.
