Provides rulemaking authority to the Department of Health to create a physician orders for life-sustaining treatment (POLST) form; requires that certain information be included in the form; provides guidelines for use of the form; authorizes certain future revisions to the form; requires a completed form to be part of a principal’s medical record; requires the department to place the form on the department’s website.
Effective Date: July 1, 2006.
