Pharmacy: Provides requirements for contracts between pharmacy benefit managers & contracted pharmacies; requires pharmacy benefit manager to ensure that prescription drug has met certain requirements to be placed on maximum allowable cost pricing list; requires pharmacy benefit manager to disclose certain information to plan sponsor; requires contract between pharmacy benefit manager & pharmacy to include appeal process.
