HB 963 (2016) -  Health Insurance Restrictions on Prescription Medication Protocols

Requires Medicaid managed care plan, insurer, or HMO to allow prescribing provider to request override of restriction on use of medication imposed through step-therapy or fail-first protocol; prohibits duration of step-therapy or fail-first protocol from exceeding time period specified by prescribing provider.
